Pain, Partnerships, and Pleasure: Coping with Endometriosis

Endometriosis is a pervasive medical condition affecting millions of women worldwide, causing chronic pelvic pain, painful menstrual periods, and painful intercourse. As a sex therapist, I have witnessed how this condition can adversely affect a woman’s sexual well-being. So, in this article, I will examine this issue in greater depth and explore various management strategies.

What Is Endometriosis?

This problem develops when the uterine lining tissue spreads outside the uterus and clings to the ovaries, fallopian tubes, or bladder, among other pelvic organs. This tissue behaves like the uterine lining, thickening and shedding during the menstrual cycle, resulting in discomfort and inflammation. Unfortunately, painful intercourse is a typical symptom of endometriosis for many women. 

The Importance of Sex Therapy

Seeking medical attention is essential when talking about endometriosis management. However, even though medical interventions can alleviate some of the endometriosis symptoms, they are not always effective and do not address the impact on a woman’s sexual well-being. So, women usually seek help from sex therapists to address this aspect of endometriosis management or learn to manage it together as a couple. 

It is important to note that managing endometriosis in relationships requires understanding  and ongoing communication

Because endometriosis can be chronic and may require continuous treatment and management, partners need to be supportive and understanding of this condition. This support includes understanding the physical limitations, such as chronic pain and fatigue. Emotional support is also essential, as endometriosis can significantly impact a person’s mental health. Partners can offer a listening ear, validate feelings, and help find resources for mental health support if needed.

Communication is also necessary when it comes to managing endometriosis in relationships. Openly discussing pain and discomfort during sex can be awkward, but it is unavoidable for couples who want to improve intimacy and reduce feelings of shame and inadequacy. Couples can work together to find more comfortable and less painful positions, experimenting with different sexual positions to find what works best. 

For example, some couples report the spooning position helpful, where partners lie on their sides, facing the same direction, with the partner behind holding the partner in front. This position can be less painful as it allows for more control over penetration depth and angle. Another position that can work well is the woman on top position, as it will enable the woman to control the depth and pace of penetration.

The Importance Of Working With A Physiotherapist

I always work closely with a physiotherapist to manage endometriosis when I see individuals or couples. I believe pelvic floor physiotherapy is an integral part of endometriosis management, as it can help reduce pelvic pain and improve sexual function. 

During physiotherapy sessions, clients may learn exercises to strengthen their pelvic floor. These exercises can improve blood flow and reduce pain. They may also receive guidance on breathing techniques, relaxation exercises, and stretches to help reduce tension in the pelvic area.

Working in a team with pelvic floor physiotherapists, I can monitor my clients’ progress and adjust their treatment plans to ensure they receive the best possible care. The work with a physiotherapist takes place while I focus on addressing the emotional and psychological impact of endometriosis on my clients. 

Living with endometriosis can be a daunting challenge for many women, affecting their physical and emotional well-being, especially in the realm of sexual intimacy. Although medical interventions can alleviate some symptoms, I believe addressing the impact on sexual health requires a holistic approach that combines medical care, sex therapy, and physiotherapy. By working together as a team, healthcare professionals can provide personalized support to help women with endometriosis improve their quality of life.

While it may not be a journey without challenges, there are effective management strategies available to help women live fulfilling lives and maintain healthy relationships
